GROUP TRANSFER FOR AN EXAM FORM
THE INITIAL INSTRUCTOR MUST:
- Contact the Lifesaving Society to get the group transfer authorization.
- Write “Group transfer” next to the candidate’s name on the initial course exam sheet. If results are submitted online: select “En attente de résultat” for the transferred candidate and write in the comment field ‘’Group transfer for (name of candidate)”.
- Help the candidate find an aquatic facility where they can write their exam within three months following the initial course exam date.
- Write the candidate’s information on a new exam sheet and check all assessed items they have successfully completed during the initial course, and complete the instructor and affiliate member sections.
- Complete the Group transfer for an exam form.
- Send the completed form, an official document justifying the transfer and the completed exam sheet to the hosting instructor. The initial instructor of the initial course must keep a copy of these documents in their personal files.
THE HOSTING INSTRUCTOR MUST:
- Ensure the transferred candidate’s integration.
- Mail the following documents along with the other candidates’ exam sheets to the Lifesaving Society. Keep a copy of these three documents for their files.
- Group transfer for an exam form
- Document justifying the group transfer
- The transferred candidate’s exam sheet
Note: It is not possible to submit online or by email any transferred candidate’s documents or results.
THE HOSTING GROUP EVALUATOR MUST:
- Complete and sign the transferred candidate’s exam sheet.
- If the candidate has successfully completed an item when assessed by the initial instructor but fails that item during the exam, the final result is a failure. In this situation, the examiner must modify the candidate’s results and initial the changes made, and fill a failure report if necessary.
Award fees will be charged to the affiliate member who offered the candidate’s initial course.
